Insurance Information

All registered Ashfield Pirates FC players are covered under the Football NSW Insurance Program.

Player insurance is administered by Football NSW and applies to injuries sustained while participating in sanctioned Football NSW activities, including matches and training.

All injury claims are lodged online via the Football NSW Claims Portal. This system allows claimants to submit forms, upload documents and track claims for faster processing.

How to Lodge a Claim

1. Visit the Football NSW website and select “Lodge Your Claim Online.”

2. Register and complete the online injury claim form.

3. Download the required forms from the Claims Portal:

   • Club Declaration Form

   • Physician’s Statement

4. Have the Club Declaration signed by Ashfield Pirates FC.

5. Have the Physician’s Statement completed by your Doctor or Specialist.

6. Upload all completed forms, receipts and supporting documents back into the Claims Portal.

All injury claims must be verified before assessment.

• Claims for players registered with a local community club are verified by the relevant Association.

• Other claims are verified directly by Football NSW.

Verification confirms that:

• The player was registered at the time of injury

• The injury occurred during a sanctioned Football NSW activity

• All information and documentation provided is accurate

Claims can only be verified once all required documentation has been uploaded. Additional information may be requested by the insurer if needed.
